What is Nurtec ODT (rimegepant) for?

Nurtec ODT (rimegepant) is a calcitonin gene-related peptide (CGRP) receptor antagonist indicated for the acute treatment of adults with either episodic or chronic migraine, with and without aura.

It is available as orally disintegrating tablets (ODT) containing 75 mg rimegepant. This is the first and only CGRP receptor antagonist that can be taken orally for the acute treatment of migraine attacks.

How does Nurtec ODT (rimegepant) work?

Rimegepant is a monoclonal antibody, a type of protein, that blocks the activity of the calcitonin gene-related peptide (CGRP) receptor. CGRP is a molecule that is involved in migraine attacks. When its levels go up, it triggers intense inflammation in the brain, causing migraine headaches. By blocking the CGRP receptor, rimegepant can relieve pain experienced during attacks.

Because it is an orally dissolvable tablet, Nurtec ODT (rimegepant) is taken up in the blood rapidly, and will reach its maximum effect in 1.5 hrs.

Nurtec ODT (rimegepant) offers an alternative to triptans, the most frequently prescribed migraine therapies, particularly for patients whose symptoms do not respond to triptans.

Clinical trials

The Food and Drug Administration (FDA), USA approval of Nurtec ODT (rimegepant) for the acute treatment of adult patients with migraine with or without aura was based on the data of Study 303 (NCT03461757).

Study 303 was a phase 3, double-blind, randomized, placebo-controlled trial to study the safety and efficacy of Nurtec ODT (rimegepant). The trial involved 1811 patients with acute migraines. Upon the occurrence of a migraine attack, patients were given a single dose of either 75 mg rimegepant or placebo.

Results

The main efficacy outcome measures were the percentage of patients who experienced relief of pain and of most bothersome symptoms (MBS) after 2 hours after taking rimegepant.

Rimegepant 75 mg caused freedom of pain and MBS at 2 hours after administration, compared to placebo.

Two hours after taking rimegepant or placebo, 19.6% of patients treated with rimegepant, as compared to 12.0% of patients treated with placebo, were free from pain. The absolute difference was 7.6% [95% CI: 3.3, 11.9].

The percentage of patients who had freedom of MBS 2 hours after dosage was 37.6% in the rimegepant group, versus 25.2% in the placebo group (absolute difference, 12.4% [95% CI: 6.9, 17.9].
