\Lonsurf is a fixed-dose combination medication that combines Trifluridine, a nucleoside metabolic inhibitor, with Tipiracil, a thymidine phosphorylase inhibitor. It is prescribed for adult patients with the following conditions:
- Metastatic colorectal cancer: The medicine can be used alone or alongside bevacizumab for patients who have already undergone treatment with fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, as well as anti-VEGF biological therapy. For those with RAS wild-type cancer, it is also used after anti-EGFR therapy.
- Metastatic gastric or gastroesophageal junction adenocarcinoma: Lonsurf is an option for patients who have previously received at least two lines of chemotherapy, including a fluoropyrimidine, a platinum agent, and either a taxane or irinotecan. If applicable, it is also used in patients who have received HER2/neu-targeted therapy.
Recommended Dosage: The recommended dosage of Lonsurf, whether used alone or with bevacizumab, is 35 mg/m² (based on the trifluridine component) taken orally twice daily with food. The maximum dose is 80 mg per administration. This regimen should be followed on Days 1-5 and Days 8-12 of each 28-day cycle. Treatment continues until the disease progresses or unacceptable side effects occur. Dosages should be rounded to the nearest 5 mg.
Warning & Precautions
- Treatment with Lonsurf tablets can cause severe bone marrow suppression, leading to neutropenia, anemia, and thrombocytopenia. Monitor blood counts before and during treatment. Dose modifications or discontinuation may be required if severe hematologic toxicity occurs.
- Trifluridine and Tipiracil can cause fetal harm when administered to pregnant women. Advise females of reproductive potential to use effective contraception during treatment and for at least 6 months after the final dose.
- Lonsurf may harm a breastfeeding infant, as it can be excreted in breast milk. Breastfeeding is not recommended during treatment with Lonsurf and for one day following the last dose. Patients should be advised to either discontinue breastfeeding or discontinue the drug, considering the drug’s importance to the mother.
- Caution is advised when administering Lonsurf (15 mg trifluridine/6.14 mg tipiracil & 20 mg trifluridine/8.19 mg tipiracil) to patients with moderate to severe hepatic impairment. Limited data is available on its use in this population, and adjustments in dosage or close monitoring may be necessary due to potential alterations in drug metabolism and increased risk of toxicity.
- Lonsurf should be used with caution in patients with moderate to severe renal impairment. Dosage adjustments may be required, and renal function should be closely monitored during treatment, as impaired renal function may lead to increased drug exposure and toxicity.
