Medical Uses
Diacomit is an anticonvulsant medicine used for the treatment of seizures associated with Dravet syndrome in patients taking clobazam who are aged 6 months and older and weighing 7 kg or more. There is no clinical data to support the use of this pharmaceutical product as monotherapy in Dravet syndrome.
Recommended Dosage: The recommended dosage is 50 mg/kg/day, administered in 2 or 3 divided doses (i.e., 16.67 mg/kg thrice daily or 25 mg/kg twice daily), depending on the patient’s age and body weight. If the exact dosage can not be achieved with available strengths, round to the nearest possible dosage, which is usually within 50-150 mg of the recommended 50 mg/kg/day. A combination of a couple of Diacomit strengths can be used to achieve this dosage. The maximum recommended total dosage is 3,000 mg daily.
Diacomit capsule must be swallowed completely with a glass of water during a meal. Avoid breaking or opening the capsules.
Diacomit Powder for Oral Suspension should be mixed in a glass of water (100mL) and should be taken promptly after mixing during a meal. To be sure there is no drug left in the glass, add a small amount of water (25mL) to the drinking cup and drink all of the mixture.
Take a missed dose as soon as possible. If it is near time for the very next scheduled dose, take the missed dose. Instead, take the next scheduled dose. Do not double the doses.
Warning & Precautions
- Diacomit 250 mg or 500 mg medication can cause somnolence. Doctors should evaluate patients for somnolence. Doctors should alert patients against engaging in hazardous activities requiring mental alertness, such as operating dangerous machinery or motor vehicles, until the effect of medicine on mental alertness is known.
- Concomitant use of medication Stiripentol with certain other existing CNS depressants, including alcohol, may be responsible for increasing the risk of sedation and somnolence.
- Stiripentol can cause decreases in appetite and weight. Nausea and vomiting may also occur a bit more frequently in Stiripentol-treated patients. Given the frequency of these side effects, the growth of pediatric patients treated with drug Stiripentol should be carefully monitored.
- The stiripentol capsule can be responsible for causing a significant reduction in neutrophil and platelet count. Hematologic testing needs to be done prior to starting therapy with Stiripentol, and then every six months.
- As with most antiepileptic medicines, Diacomit should generally be withdrawn gradually to reduce the probability of higher seizure frequency and status epilepticus. At junctures where rapid withdrawal of Diacomit is needed (e.g., in the setting of serious side effects), apt monitoring is required.
- Phenylalanine can be hazardous to those with phenylketonuria. Diacomit for oral suspension contains phenylalanine, a component of aspartame. Each 250 mg and 500 mg packet contains 1.40 mg and 2.80 mg phenylalanine respectively. Before prescribing it to a patient with Phenylalanine, consider the combined daily quantity of phenylalanine from entire sources, including Diacomit for oral suspension. Capsules do not contain phenylalanine.
- AEDs, including Diacomit 500mg or 250mg, may increase the risk of suicidal thoughts or behavior in those taking these medicines for any indication. Those treated with any AED for any indication need to be assessed for the emergence or worsening of depression, suicidal thoughts or behavior, or any unusual changes in behavior or mood. Patients should notify their health specialist if they become pregnant or intend to become pregnant during Stiripentol therapy. Encourage patients to enroll in the NAAED Pregnancy registry in case they become pregnant. Patients should notify their health specialist if they are breastfeeding or intend to breastfeed during therapy.
