What is Wakix (pitolisant) for?
Wakix (pitolisant) is a histamine-3 (H3) receptor antagonist/inverse agonist used to treat excessive daytime sleepiness or cataplexy (sudden loss of muscle strength) in adults with narcolepsy.
It is available as an oral tablet, containing 4.45 mg and 17.8 mg pitolisant.
How does Wakix (pitolisant) work?
How Wakix works exactly is not yet understood, but it is thought to affect histamine. It increases histamine levels in the brain. Histamine is found naturally in the brain and helps the body to stay awake and the muscles to have a stable tone throughout the day.
The active ingredient in Wakix, pitolisant, can bind to receptors in the brain called ‘histamine H3’. By doing so, Wakix can increase the activity of brain cells and help keep the body awake.
Where has Wakix (pitolisant) been approved?
Wakix (pitolisant) was approved by:
- The European Medicines Agency (EMA) on March 31, 2016 to treat EDS and cataplexy in narcolepsy.
- The Food and Drug Administration (FDA), USA:
- On August 15, 2019 to treat EDS in narcolepsy.
- On October 13, 2020 to treat cataplexy in narcolepsy.
- Health Canada on June 9, 2021 to treat EDS and cataplexy in narcolepsy.
Wakix (pitolisant) was granted Orphan Drug Designation by the EMA.

How is Wakix (pitolisant) taken?
The standard dosage is:
- 17.8 mg to 35.6 mg taken orally once per day, as soon as you wake up.
Your treating doctor will determine the right dosage for you via a process called titration: you will start at a lower dose and increase the dose gradually to find the right dose.
Take the capsules at the same time every day. If you miss a dose, skip the dose and take it the following day as soon as you wake up.
It is important to know that it may take some time for Wakix (pitolisant) to start to have effect, for some patients it may take up to 8 weeks.
If you have moderately reduced liver function or kidney problems, your maximum dose should be 18 mg per day. You should not use Wakix (pitolisant) if you have severe liver disease.

Use in a specific population
Tell your treating doctor if you are pregnant or planning to become pregnant.
Wakix (pitolisant) can decrease the effectiveness of some medicines, including hormonal birth control methods. Patients should use an alternative non-hormonal birth control method during treatment and for at least 21 days after stopping treatment.
Tell your treating doctor about all the medicines you take and of all your medical conditions, including if you have heart rhythm irregularities, were born with a heart condition, or the levels of electrolytes in your blood are too high or too low.

Clinical trials
Excessive daytime sleepiness
The European Medicines Agency (EMA) approval of Wakix (pitolisant) for treatment excessive daytime sleepiness (EDS) in adults with narcolepsy was based on two studies Study 1 (NCT01067222) and Study 2 (NCT01638403).
These trials were two multicenter, randomized, double-blind, placebo-controlled studies that included a total of 261 patients. Patients were randomized to receive Wakix or placebo once daily for 8 weeks.
Results
The main endpoint of the studies was the measure of how sleepy patients felt during daytime, assessed using the Epworth Sleepiness Scale or ESS.
Wakix was effective in reducing EDS in patients with narcolepsy. At the end of the study:
- The ESS score with Wakix was 12.4 versus 15.5 with placebo in Study 1
- The ESS score with Wakix was 13.3 versus 15.5 with placebo in Study 2
Wakix showed greater improvement in ESS score compared with placebo. At the end of the study:
- Wakix had reduced the ESS score by 6 points, compared to the 2.9 points reduction seen with placebo in Study 1.
- Wakix had reduced the ESS score by 5 points, compared to the 2.8 points reduction seen with placebo in Study 2.
Cataplexy
The European Medicines Agency (EMA) approval of Wakix (pitolisant) for treatment of cataplexy in adults with narcolepsy was based on two studies: Study 1 (see EDS) and Study 3 (NCT01800045).
Study 3 was a 7-week, multicenter, randomized, double-blind, placebo-controlled study that involved a total of 154 patients. In both trials, patients were treated with either Wakix or placebo once daily.
Results
The main endpoint of the studies was the number of weekly cataplexy attacks, which are defined as sudden muscle weakness often triggered by emotions. One trial measured the number of weekly cataplexy attacks and the other trial measured the number of daily cataplexy attacks
Wakix nearly halved the number of cataplexy attacks per week compared with placebo:
- The number of weekly attacks was reduced by 6.8 with Wakix versus 2.8 with placebo in Study 3
On average, patients taking Wakix had approximately 8 weekly cataplexy attacks.
Wakix significantly lowered the number of daily cataplexy attacks compared to placebo.
